Referring Inpatient Agencies
Partnering with Referring Agencies
The Precourt Healing Center is committed to providing comprehensive, compassionate inpatient behavioral health services for adolescents and adults. We deeply value the collaborative partnerships that make seamless patient transitions possible. We are dedicated to engaging with appropriate referring agencies to ensure a transparent and efficient admissions process.
Who We Serve
The Precourt Healing Center is equipped to support:
- Adolescents (ages 12–17)
- Adults (18 and older)
We treat a broad spectrum of behavioral health needs, including patients experiencing depression, anxiety, mood disorders, or co-occurring challenges. Please note: Precourt Healing Center is not a detox or SUD treatment facility.
Each referral is evaluated individually to ensure safety, clinical appropriateness, and availability of services.
Referral Process
To refer a patient to the Precourt Healing Center:
- Complete the current admissions checklist linked below. The intake packet requirements are frequently updated to reduce administrative barriers for patients and providers, while also complying with regulatory requirements. Please do not download and use past versions of our checklist/packet, as only the most current version will be accepted.
- Submit the completed packet, including:
- Demographic and insurance information
- Medical history
- Consent for treatment
- If needed, ensure the patient has been medically cleared prior to admission.
- Medical clearance must be completed by a licensed healthcare provider, confirming that the patient is stable and does not require acute medical intervention.
You can submit the completed packet by email to PHCintake@vailhealth.org or by fax to (970) 470-6511.
Our intake and admission team will review all submissions promptly to determine clinical fit and bed availability.
Transportation
At this time, the Precourt Healing Center is not equipped to provide patient transport. We ask that referring agencies coordinate secure medical transport services as needed for patient transfers.
