La navegadora es el enlace entre todos los componentes de la atenci\u00f3n; brindando informaci\u00f3n de manera transparente al paciente y a su familia, y entre los proveedores de atenci\u00f3n en cada entorno asistencial que el paciente utilice.<\/div>\n<div>\u00a0<\/div>\n<div><strong>Lo que har\u00e1s:<\/strong><\/div>\n<div>\n<ul>\n<li>Consultar con los pacientes por tel\u00e9fono o en persona para determinar necesidades espec\u00edficas y determinar derivaciones a la atenci\u00f3n m\u00e9dica.<\/li>\n<li>Construir una relaci\u00f3n centrada en el paciente y la familia.<\/li>\n<li>Proveer evaluaci\u00f3n de admisi\u00f3n inicial, tamizajes y evaluar las barreras para la atenci\u00f3n.<\/li>\n<li>Reportar al equipo.<\/li>\n<li>Asistir y\/o revisar las citas con proveedores; reforzar la educaci\u00f3n y la informaci\u00f3n proporcionada por los cuidadores.<\/li>\n<li>Desarrollar y entregar un plan de atenci\u00f3n con un resumen del tratamiento a los pacientes y\/o firmar consentimientos y rechazos, y reforzar la educaci\u00f3n sobre los tratamientos aceptados o rechazados.<\/li>\n<li>Organizar un plan de atenci\u00f3n basado en la evidencia y presentarlo a los pacientes para su discusi\u00f3n.<\/li>\n<li>Hacer el seguimiento necesario con el paciente y los proveedores.<\/li>\n<li>Mantener una relaci\u00f3n centrada en el paciente con este y su familia durante el curso de la atenci\u00f3n y en el periodo posterior a la misma seg\u00fan sea necesario.<\/li>\n<\/ul>\n<\/div>\n<div><strong>Esta descripci\u00f3n no pretende ni debe interpretarse como una lista exhaustiva de todas las responsabilidades, habilidades, esfuerzos o condiciones de trabajo asociadas con el puesto.
